2nd Dose Date: Sunday May 23rd 2021.  More information will be provided when you arrive for your 1st dose

PLEASE READ Below Instructions CAREFULLY

On the day of appointment it is important to show up AT THE TIME of your appointment.  When you arrive early, you will create backlog for others who are scheduled at the later time.  Please help us to keep the process moving by arriving only at your appointment time.

Instructions:

• When you arrive, park your car
• You will need to register first by the entrance of the golf course where you will be handed short screening questions (To verify any medical history that might have changed from the time you filled out the form to the vaccination day)
• Take the screening form to the vaccinators to get your vaccine
• You will be then moved to waiting room area for 10-15 mins monitoring before you can go back to your car 
• Handicap accessible parking area available and will have ability vaccinate them in their car as well.

PLEASE BRING: PHOTO ID & Copy of your PRESCRIPTION INSURANCE CARD (NOT YOUR HEALTH INSURANCE CARD)
